Informed consent for telehealth services

This consent explains telehealth, its benefits and limitations, and your rights. You will sign a copy of this consent before your first visit. The version below is for your reference.
What telehealth is
Telehealth is the delivery of health care services using telecommunications technology — for example, secure messaging or video conferencing — when you and your clinician are not in the same physical location. Through Sinad Labs, your visit may be conducted asynchronously by message, synchronously by video, or by a combination.
Potential benefits
- Convenient access to a licensed clinician without travel or waiting rooms
- Faster initial response for many common conditions
- The ability to be seen from your home
- Continuity of care through messaging follow-up
Potential risks and limitations
- Without a physical exam, your clinician may not be able to make a definitive diagnosis or may need to refer you for in-person care.
- Technology may fail or messages may be delayed.
- Despite reasonable security measures, electronic transmissions are never 100% secure.
- Some conditions are not appropriate for telehealth.
- Some prescriptions cannot be issued via telehealth under federal or state law.
Alternatives
You may always choose to see a clinician in person rather than by telehealth. Your clinician may also recommend in-person care if it is more appropriate for your concern.
Your rights
- You have the right to withhold or withdraw consent at any time without affecting your right to future care.
- You have the right to confidential treatment of your information, as described in our Notice of Privacy Practices.
- You have the right to be told who your clinician is, including their credentials and licensure.
- You have the right to ask questions about any recommendations and to seek a second opinion.
Emergencies
Telehealth is not appropriate for emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. For mental-health crises, call or text 988.
Recordings
Sinad Labs does not record video visits. Messaging visits are documented as part of your medical record. Your clinician may take notes during a video call as part of standard documentation.
