This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Who this notice covers

This notice applies to the protected health information ("PHI") created or received by the network of independent clinicians and clinician-owned professional entities that provide care through the Sinad Labs platform. Sinad Labs, Inc. supports these clinicians as a business associate under HIPAA.

How we may use and disclose your PHI

For treatment

Your clinician may use your PHI to provide care, and may share it with other clinicians, pharmacies, or labs involved in your care.

For payment

We may use and share your PHI to bill and receive payment for services. For example, we may share information with our payment processor to charge your card.

For health care operations

We may use and share your PHI for activities necessary to run the practice, improve quality, and ensure care is appropriate.

As required by law

We will share PHI when required to do so by federal, state, or local law.

For public health and safety

We may share PHI to report disease, injuries, certain reactions to medications, or to prevent or reduce a serious threat to anyone's health or safety.

Other uses

Other uses and disclosures not described in this notice will be made only with your written authorization. You may revoke an authorization at any time, in writing, except to the extent we have already acted on it.

Our duties

We are required by law to maintain the privacy of your PHI, to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

Changes to this notice

We may change this notice. We will post the current version on this site and make paper copies available on request.
